First Visit
Brief Paperwork
Upon entering the office, we have some brief paperwork for you to fill out. The paperwork provides us with general information about yourself and your condition.
Please click here to download and print our new patient forms which you can complete prior to your first visit to our office.

Once the paperwork has been completed, you will have a consultation with the doctor to discuss your health-related problems, concerns, as well as potential treatment options. When a patient seeks chiropractic health care and we accept a patient for such care, it is essential for both to be working towards the same objective. Chiropractic care utilizes adjusting techniques, which are specific application of forces to facilitate the body’s correction of vertebral subluxation. The vertebral subluxation is characterised by a misalignment or change in mobility of one or more vertebra. Subluxation causes an alteration of nerve function resulting in a lessening of the body’s innate ability to express its maximum health potential. History & Examination
In order to determine and define the areas of your body that are not functioning correctly based on vertebral subluxations and the impact on the nervous system, we will be asking you specific health history questions. We will then provide a sophisticated and comprehensive examination and specialized testing to be able to show you the underlying nervous system. Testing includes nutritional assessments, digital radiography, and advanced thermography and surface EMG scanning – all focused on finding and changes and/or damage to the spine and nervous system.

X-Ray Studies
Your specific condition may require us to take x-rays to either rule out more serious conditions or assist us in developing the most effective treatment plan for you. We utilize the most advanced digital Xray technology, which is the BEST quality with the LOWEST radiation exposure.

Home Instructions
Prior to leaving, patients will be given instructions on certain activities or procedures to be conducted at home. This may include ice or heat application instructions, avoidance of certain activities or positions, as well as home exercises and/or stretches.

Schedule Your Next Appointment – Review Your Test Results
You will be given a convenient appointment time for your follow-up visit. Generally speaking, patients are seen within 1-2 days and are provided with a complete review of findings. We review all the tests done and educate you on your specific condition or health goals and options. Treatment options for your condition will also be discussed during this time.
